敗血症とプロカルシトニン(1)~概要,精度~

1.プロカルシトニンとは?
■2010年のPierrakosらのレビュー[1]によれば,敗血症では178バイオマーカーが研究されていると報告されているが,現時点では高い確実性をもったバイオマーカーはない.また,バイオマーカーを評価する上で,その患者が敗血症であることを臨床診断しなければならないが,1991年のACCP/SCCMによるSIRS基準[2],2001年のSCCM/ACCP/ESICM/ATS/SISによる基準[3]が定められてはいるものの,その精度や予後に差はなく[4,5],1991年診断基準(SIRS基準)の精度は感度94.6%,特異度61.0%,2001年診断基準の精度は感度96.9%,特異度58.3%であり,AUCはそれぞれ0.778,0.776と有意差がなかった.よって,現時点で敗血症診断にゴールデンスタンダードは存在しないため[1,6],バイオマーカーの精度評価が困難となっている.理想的バイオマーカーとして,簡便,迅速,客観性,再現性,感度・特異度が良好,患者重症度を鋭敏に反映,安価,などが要求される[7]

■現在敗血症領域で精度が比較的高く,臨床応用されている代表例としてプロカルシトニン(PCT:procalcitonin)がある.PCTは米国ではあまり推奨されていないが,欧州を中心に研究が進み,本邦においても敗血症疑いに対して保険点数320点で承認されている.

■PCT検査は湾岸戦争の際に肺腺癌マーカーとして研究開発されたが,1993年にAssicotら[8]が敗血症関連蛋白として報告した.PCTは116個のアミノ酸で構成される分子量130kDa,通常は甲状腺C細胞でカルシトニンの前駆体として産生されている.しかし,細菌感染症による敗血症や重篤な炎症反応が生じた場合は,TNF-αなどの炎症性サイトカインの増加により,肺・肝臓・腎臓・筋肉などの全身の細胞で産生される.その一方で,ウイルス感染症や真菌感染症では増加しにくいことが知られている[9,10].また,PCTは白血球などの血球成分からはほとんど分泌されない[9].すなわち,ステロイドや抗癌剤などの白血球機能に影響を与え易い薬剤を使用しているような状況下であっても,細菌感染症の場合はPCT上昇は妨げられない[11]

■反応時間はCRPが6時間,PCTが2-3時間,ピークはCRPが48時間,PCTが24時間,半減期はCRPが4-6時間,PCTが20-24時間である.なお,PCTは透析膜の影響を受けうる.Montagnanaらの44例の観察研究[12]では,透析において高流量膜では透析前後でPCT値は有意に低下し(0.50→0.26ng/mL, p=0.005),低流量では有意な変化はみられなかった(0.41→0.42ng/mL, p=0.863).本邦の透析は原則として低流量であるため,PCT値は影響を受けにくいと推察される.

■以下ではPCTの精度と活用法に関する知見をまとめた.なお,PCTの研究は非常に多く,PubMed検索では2363件がヒットし,RCTは87報,メタ解析は30報におよぶ.すべては紹介できないので,メタ解析を中心に,比較的新しいもの,研究内容が特筆すべきものに限って紹介する.

2.プロカルシトニンの精度
■PCTの精度についてのメタ解析14報[13-26]を以下にまとめた.

■これらの報告ではPCTの感度は0.59-0.96,特異度は0.43-0.91,AUROCは0.61-0.91とバラつきがあるが,おおむね0.7台といったところで,この数字だけ見ればCRPに比して特段優れているわけではない.とりわけ急性虫垂炎,肺炎随伴性胸水に関してはPCTは精度は低く推奨されないだろう.ただし,研究によってPCTカットオフ値に違いがあり,一概には評価しにくい面がある.

■健常人ではPCTの正常値は0.05ng/mL未満であり,特異度を高くとるのであれば,0.5ng/mLを越えると異常,0.5-2.0ng/mLが境界領域で,2ng/mLを超えると全身細菌感染症が強く疑われ,10ng/mL以上では重症敗血症,敗血症性ショックなど重症患者であることが多いとされる.しかし,Tangら[23]の報告を見ても分かる通り,敗血症とSIRSの鑑別におけるPCTの精度は決して芳しくない.

■本邦からもAikawaらの多施設共同前向きコホート研究(7施設245例)が1報あり[27],細菌感染の診断でのPCTのカットオフ値を0.5ng/mLとすると,感度0.64,特異度0.86,AUROC 0.84であり,エンドトキシン(AUROC 0.60),IL-6(AUROC 0.77),CRP(AUROC 0.78)に比して優れているという結果であった.

■カテーテル血流感染症(CRBSI)についてはTheodorouらの46例の観察研究がある[28].この報告では,CRBSIが疑われたICU患者において,PCTカットオフ値0.70としたときのCRBSI診断は感度92.3%,特異度100%であった.また,4日以内の値からPCT上昇幅が0.20以上のときの陽性予測値は96%であった.

■担癌患者119例,非担癌患者100例の検討では,Rapid Response Teamが契機となったPCTの測定は,感染症によるICUへの移送の必要性を予測するとしている[29]

■Quらは重症急性膵炎患者71例のRCTにおいて,感染の臨床徴候・症状とPCTにより抗菌薬投与開始・終了を決定する方法は,予防的抗菌薬を2週間投与する方法に比して有意に抗菌薬投与期間と入院期間を短縮し,コストを減少させたと報告している[30]

■蜂窩織炎,細菌性髄膜炎,コアグラーゼ陰性ブドウ球菌菌血症では偽陰性をきたすことがある[10,31,32].また,学会ポスター発表においては,グラム陽性菌感染症ではPCTの感度が落ちるとする報告も散見されている.一方,遷延するショック,心肺停止蘇生後,自己免疫性疾患,神経内分泌腫瘍,誤嚥,膵炎,敗血症,外傷,熱傷,熱中症,手術,甲状腺髄様癌でPCT上昇の偽陽性が報告されている[33-35]

■PCTの精度の報告では,異常値であっても高値の場合と低値の場合で解釈が難しくなることが想定される.敗血症が疑われる症例においてはPCTが高値であることをもって抗菌薬を投与する方法は比較的安全と推察されるが,PCTが低値であることをもって抗菌薬を投与しないとするのはリスクが高まる可能性がある.万能ではないので,PCTはあくまでも参考所見にとどめるべきであろう.逆に,細菌感染ではない重症病態の際は,むしろPCTの低さが感染症除外に役立つ可能性がある.そのときどきの状況に応じてPCTをうまく使えば有用なツールであると考える.

※当院ではNPPVを含む人工呼吸管理を要する間質性肺炎急性増悪においてはPCT計測は抗菌薬必要性有無を判断する有用なツールと考えている.自験例ではPCT値はほぼ0.1ng/mL未満の症例がほとんどであり,0.1ng/mL未満の症例5例では,抗菌薬を投与せずステロイドを投与してもその後の感染症増悪と思われるエピソードは1例も認めていない.

※PCTに続いて,IL-6も保険承認される可能性があるが,PCTとの違いについて聞かれることがある.Jekarlら[36]は,177例のSIRS患者の解析を行い,敗血症においてPCTは診断マーカー,IL-6は予後予測マーカーとしている.

PCTは敗血症診断マーカーとして位置づけがあるが,私は敗血症の診断目的ではPCTは計測していない.私の診断目的での主な用途は,重症病態での計測で細菌感染症を除外することにあり,0.125ng/mL程度をカットオフとしている.腫瘍熱や間質性肺炎急性増悪において特に使用している.逆に軽症例では判断が難しくなるため計測していない.
