敗血症とせん妄(2) ~せん妄の予防~
■鎮静薬のふみこんだ話はボリュームがかなり大きいので,別のまとめで扱うことにし,今回はガイドラインに沿った話でまとめた.起こったせん妄に対する治療は重要だが,まず予防することである.
Summary
・せん妄の危険因子は認知症・高血圧・アルコール依存症の既往,入院時の重症度,昏睡,ベンゾジアゼピン系薬の使用である.
・早期リハビリテーションはせん妄予防となる.
・せん妄予防目的での薬物療法は,ハロペリドール,非定型抗精神病薬,デクスメデトミジンなどが有効な可能性がいくつかのRCTで示されているが,現時点ではエビデンスが乏しく,推奨されていない.
・ICUにおける睡眠障害はせん妄の大きな要因であり,光・音などのICU環境の制御により睡眠の質と量を改善することでせん妄を予防できる可能性がある.

5.せん妄の予防
(1) せん妄の危険因子
■ICUに入室した患者のせん妄の危険因子は以下の通りである[6]

① 既存の認知症[36-38]
② 高血圧の既往[13,39]
③ アルコール依存症の既往[13,38]
④ 入院時点で重症度の高い疾患[13,34,36,40]
⑤ 昏睡[13]
⑥ ベンゾジアゼピン系薬の使用[41]
■せん妄発症に関与している生体因子は以下の通りである.
① 神経伝達物質の異常:アセチルコリンの減少,ドーパミン,セロトニン,GABA,グルタミン酸,ノルアドレナリンの増加
② びまん性脳神経障害:脳虚血,脳血流低下,高血糖,低血糖,血管内皮障害および微小血栓形成
③ 内分泌機能:コルチゾールなどの副腎皮質ホルモン,甲状腺ホルモン
④ 不眠
⑤ 代謝異常:高ナトリウム血症,高カルシウム血症,高浸透圧,尿毒症
■せん妄を予防するにはまず上記に示したリスク因子のうち可能なものは回避もしくは治療することが基本原則である.

(2) 早期リハビリテーションによるせん妄予防
■成人ICU患者では,せん妄の発現抑制と期間短縮のために,可能な限り早期モビライゼーションを促すことが推奨される.

■Schweickertらは,人工呼吸器を装着した成人患者104例で,1日1回の鎮静中断に,早期モビライゼーション(理学療法・作業療法)を加える介入群と加えない対照群を比較したRCT[42]を行ったところ,28日間の観察で,介入群で有意にせん妄期間が短縮し(中央値2.0日[IQR 0.0-6.0] vs 中央値4.0日[IQR 2.0-8.0]; p=0.02),人工呼吸器を装着していない期間も有意に短縮した(23.5日[7.4-25.6] vs 21.1日[0.0-23.8]; p=0.05).

■Needhamらは,人工呼吸器を装着した内科ICU患者を対象として,集学的チームにより深鎮静を減じ,モビライゼーションを促進する質改善プロジェクトを施行し,57例での前後比較研究を行った[43].その結果,プロジェクト開始後でベンゾジアゼピン系薬の使用,鎮静薬の用量が有意に減少し,鎮静・せん妄状態が改善した.2013年にも同様に,早期モビライゼーションの質改善プログラムの導入がICU在室期間,入院期間の短縮とともに鎮静薬の必要度とせん妄を減少させたと報告している[44]

(3) 薬物療法によるせん妄予防
■せん妄治療としての薬物療法は推奨があるものの,これらを予防目的で使用することはガイドラインでは推奨されていない.具体的にはハロペリドール,非定型抗精神病薬(セロトニン・ドパミン遮断薬SDA;リスペリドン,ペロスピロン,ブロナンセリン,パリペリドン/多元受容体作用抗精神病薬MARTA;オランザピン,クエチアピン,クロザピン/ドパミン受容体部分作動薬;アリピプラゾール)であるが,ICU患者において十分なサンプルサイズ・エフェクトサイズの質の高い研究が存在しないためである.一方,中等度以下の質ではRCTがいくつか存在する.

■Wangらは,65歳以上の心臓以外の術後ICU患者457例に対してハロペリドールの投与を検討したプラセボ対象二重盲検RCTを行い[45],ハロペリドール群がプラセボ群より,術後7日までのせん妄発生率が有意に低く(15.3% vs 23.2%; p=0.031),せん妄発症までの平均期間は有意に長く(6.2日 vs 5.7日; p=0.021),せん妄のない日数は有意に長く(6.8日 vs 6.7日; p=0.027),ICU在室期間が有意に短縮した(21.3時間 vs 23.0時間; p=0.024).Girardらは,6つのICUの人工呼吸器患者101例に対して,ハロペリドール,ジプラシドン,プラセボを比較した二重盲検RCTを行い[46],3群間でせん妄・昏睡の発生や人工呼吸器装着期間,入院期間などに有意差はみられなかった.Prakanrattanaらは,心臓手術を受けた患者126例に対して術後のリスペリドン投与を検討したプラセボ対象二重盲検RCTを行い[47],リスペリドン群がプラセボ群より有意にせん妄発生率が少なかった(11.1% vs 31.7%; p=0.009; RR 0.35; 95%CI 0.16-0.77).また,RCTではないが,van den Boogaardらは,せん妄発症リスク50%以上の患者において,予防的低用量ハロペリドールの投与を検討した476例前後比較研究を行い[48],せん妄発生率(65% vs 75%; p=0.01),せん妄のない期間(20日 vs 13日; p=0.003 )が有意に改善している.

■せん妄リスクを有する術後患者でのせん妄予防を検討した5報RCT(ハロペリドール3報,リスペリドン1報,オランザピン1報),1491例のメタ解析[49]では,せん妄発生率を約半分に減少させる(RR 0.51; 95%CI 0.33-0.79; heterogeneity, p<0.01, random effects model)と報告している.

■デクスメデトミジンをせん妄予防目的で用いることもガイドラインでは有力なエビデンスがないとして推奨していない.Shehabiらは,心臓術後患者306例において,デクスメデトミジンとモルヒネを比較した二重盲検RCT(DEXCOM study)を行っている[50].せん妄発生率はデクスメデトミジン群が少ない傾向であるものの統計学的有意差はなかった(8.6% vs 15.0%; RR 0.571; 95%CI 0.256-1.099; p=0.088).しかし,せん妄期間はデクスメデトミジン群の方が3日間有意に少なかった(2日vs5日; 95%CI 1.09-6.67; p=0.0317).サブ解析では,IABPを施行された患者(20例vs25例)では有意にデクスメデトミジン群でせん妄発生率が少なかった(15% vs 36%; RR 0.416; 95%CI 0.152-0.637; p=0.001).また,デクスメデトミジン群で有意に抜管が早かった(RR 1.27; 95%CI 1.01-1.60; p=0.040, log-rank p=0.036).

(4) せん妄予防のための睡眠の質を改善するICU環境からのアプローチ
■非薬物的なせん妄予防としては,光・音をコントロールするための方策をとる,患者ケアを一括して行う,睡眠サイクルを障害しないように夜間の刺激を減らすなど環境を最適に整えることにより,睡眠を促すことがガイドラインで推奨されている.睡眠中断はヒトにとって有害であることが知られており,睡眠障害はICU患者でよく見られる[51,52].ICU環境(音,光,身体刺激)による覚醒が非常に多く,レム睡眠となるのは稀であり,完全な睡眠サイクルが得られるICU患者は少ない[51,53-56].睡眠障害は組織修復や細胞性免疫を障害し,治癒反応に影響を与える可能性がある[57].重症患者においては,睡眠障害はせん妄を誘発し[58-62],精神的ストレスを増大させる[63,64].しかしながら,ICUの睡眠科学はここ10年であまり進展していない.

■人工呼吸器を装着したICU患者には連続的な睡眠期間がほとんどないことも報告されている[65,66].以上より,ガイドラインではICUのルーチンケアを避け,光を落とし,音を減じるなどして,患者の睡眠を促進する時間を設けるべきであると提案している.しかし,エビデンスはまだ乏しい状況にある.

■アイパッチや耳栓がICU患者の睡眠の質を改善し[67],耳栓によりせん妄が減じたとするRCTもある[68].夜間のICUの音を制御することで,音や睡眠を妨げるケアが有意に減少し,睡眠の質と量も有意に改善することが報告されている[69,70]
